signals - May 10, 2005 -
General Motors Corp. is recalling 305,729 sport utility vehicles, including the Chevrolet TrailBlazer, to fix a
faulty turn signal that can flash like a hazard warning light. The recall, mostly covering vehicles sold in the
United States, includes the 2003-2004 model year GMC Envoy, Oldsmobile Bravada as well as the Isuzu
Motors Ltd.'s Ascender, which is built at a GM plant. Read

Honda Announces Recall of ATVs -
April 14, 2005 -
Honda is recalling about 200,000 2004-2005 FourTrax All-Terrain Vehicles (ATVs). The steering rods can
separate, causing the driver to lose steering control. This could cause the ATV to crash and pose a risk of
serious injury or death. The ATVs are red, olive, yellow or blue. Read

Yamaha Announces Recall Reminder for
ATVs - April 13, 2005 -
Yamaha is recalling about 86,000 Raptor YFM660R ATVs. Vibration from the engine and driveline during
long periods of high rpm usage could cause the rear master cylinder reservoir to agitate resulting in brake
fluid aeration. A small amount of air bubbles could enter the master cylinder adversely affecting rear brake
performance. Model year 2001-2004 Raptor YFM660R ATVs are involved in this recall. The 2001 and 2002
ATVs are either blue and white or black and white; the 2003 ATVs are either blue and white, black and
red, or black with blue flame graphics; and the 2004 ATVs are either blue and white or red and white.

Road Motorcycles and ATVs - April 13, 2005 -
Yamaha is recalling about 2,755 PW80 and TT-R90 Off-Road Motorcycles and 850 Grizzly 125 ATVs. The
rear brake shoe material could separate due to improper adhesive curing, adversely affecting rear braking
performance. The ATVs are either hunter green, steel blue, or camouflage. The off-road motorcycles are

Bombardier is recalling about 2,127 DS650 All Terrain Vehicles (ATVs). The rear axle bearings may have
not been sufficiently greased. This may cause a breaking of the bearing which could make the ATV

Suzuki Announces Recall of 2004-2005
Vinson and 2005 KingQuad All-Terrain Vehicles (ATVs) - March 16, 2005 -
Suzuki is recalling about 30,000 2004-2005 model year Vinson and 2005 KingQuad ATVs. Water can enter
the throttle lever case and freeze, causing the throttle lever to become stuck and fail to automatically return
to the idle position when the rider releases the throttle. If the throttle lever does not automatically return to
the idle position when the rider releases the throttle, the ATV will not slow down as the rider expects. The
rider could lose control of the ATV and an accident resulting in injury or death could occur. These ATVs are

Kawasaki Announces Recall of Kawasaki
Prairie 700 ATVs - March 14, 2005 -
Kawasaki Prairie is recalling about 1,140 700 4x4 all-terrain vehicles (ATVs). The age
recommendation warning label is missing. Children under 16 years old riding these adult-size ATVs can
suffer serious injury or death. The recalled Kawasaki ATVs include the Prairie KVF700-D1 4x4
model, which were manufactured between June 2004 and February 2005. The ATVs come in green and
red and have the words "Kawasaki" and "Prairie" printed on each side of the vehicle’s fuel tank. Read

Polaris Announces Recall of '900 Fusion,' '900
RMK' and '900 Switchback' Snowmobiles - March 14, 2005 -
Polaris is recalling about 11,560 "900 Fusion" "900 RMK" and "900 Switchback" Snowmobiles. The
snowmobile wiring harness may be routed in a way that could disconnect the engine stop switch and the
throttle safety switch connector. If this connector becomes unplugged, the engine stop and the throttle

Children's Folding Chairs - April 29, 2005 -
Atico International USA Inc., of Fort Lauderdale, Fla. is recalling about 1.5 million children's folding chairs.
The chair’s safety lock can fail, allowing the chair to collapse or fold unexpectedly. Children’s fingers can
become caught or entrapped in the hinge and slot areas of the chair, posing a pinch or cut hazard. The
recalled folding chairs are made of metal tubing with a padded seat. They were sold in red, blue, yellow
and green colors either individually or as part of a set consisting of a table and four chairs. Each chair is
about 22-inches high, 10-inches wide, and about 11-inches deep. "IMPORTED BY: ATICO
INTERNATIONAL USA, INC." is written on a label located underneath the seat of some of the chairs. Read

Poles - April 13, 2005 -
Zebco is recalling about 1.5 million Children’s Fishing Poles. The paint on the rods of these fishing poles
contains lead. Lead is toxic if ingested by young children and can cause adverse health effects. The
recalled fishing poles are brightly colored and feature pictures of the following cartoon characters on the
reels: from Nickelodeon’s SpongeBob Squarepants® are SpongeBob, Patrick Star and Sandy Cheeks;
Nick Jr.’s Dora the Explorer®; Disney’s Tigger; and the cast of Nickelodeon’s Rocket Power®.
"ZEBCO®" and "Floating Catch ‘Em Kit" are written on the handles of these poles with the
exception of the Rocket Power poles. The Rocket Power poles have a two-piece rod, were sold with

Pacifiers - April 07, 2005 -
Solar is recalling about 24,000 pacifiers. The nipple can detach from the base, posing a choking hazard to
young children.
The recalled pacifier necklace consists of a 28-inch multicolored cord with a 3-inch plastic pacifier that
comes in assorted colors. A hole at the tip of the nipple is used as a blow hole for the whistle. The pacifier
handle operates as the on-off button for the flashing light on both pacifiers. "Flashing Pacifier" or "2-in-1

buttons of the karaoke player contains excessive lead, posing a lead poisoning hazard to young children.
Lead poisoning in children is associated with behavioral problems, learning disabilities, hearing problems,
and growth retardation. This recall includes the Nu-tronix Karaoke Cassette Player and Recorder with digital

Chairs, Replacement Seats - April 06, 2005 -
L.L. Bean is recalling about 3,500 director’s chairs and replacement director’s chair padded seats. The
wooden dowels in the fabric seats are too small and can break or slide out of the fabric sleeves; the fabric
seat can tear from the wooden seat frame; and the chair frame also can break at the backrest upright, the
armrest and at the leg axis. These failures can cause the chair’s occupant to fall to the floor, and possibly
sustain injuries. The recalled director’s chair is a folding chair with a solid natural-colored wood frame. They
have cotton duck fabric seats and backs in navy, red, green and natural colors. The seats are padded and
unpadded, and replacement padded seats were sold separately. The recalled director’s chair includes

Lithonia Lighting is recalling about 120,000 High Intensity Discharge (HID) light fixtures with acrylic
reflectors. The cord of the light fixture can drip plasticizer fluid onto the light fixture, which can degrade the
acrylic reflector, causing it to crack and fall. Falling pieces of acrylic can injure a person below the fixture.
These are indoor HID light fixtures with acrylic reflectors and cords. They are generally used in locations

Laerdal Medical Corporation, Wappingers Falls, New York, a subsidiary of Laerdal Medical AS, Stavanger,
Norway, announced that it is voluntarily recalling all lots of CM 100-Heartstart Adapter Cable, Cat. No.
920650 (Adapter Cable). Wires within the defibrillator adapter cables are susceptible to breakage. Laerdal
has received reports regarding incidents of wire breakage in the Adapter Cable that prevented delivery of
defibrillation shocks. The test method described in the defibrillator instructions for use will not detect
internal breaks in the adapter cables. Use of these Adapter Cables should be discontinued. Read

Nationwide Recall of Moi-Stir Oral Swabsticks - April 22,
2005 -
Kingswood Laboratories, Inc., Indianapolis, IN, is initiating a nationwide recall of Moi-Stir Oral Swabsticks.
Some of the swabsticks have been found to contain molds including Aspergillus and Penicillium which
could result in respiratory infections. Moi-Stir Swabsticks are oral swabs used to moisten the mouths of
patients. The swabsticks are packaged as three sticks in a white foil pouch, with "Moi-Stir" on the front of the
pouch, in green lettering. The recall includes Moi-Stir Swabsticks with lot numbers A2, 1193, 1209, 1233,
1260, and 1725. Lot numbers are marked on each case, carton, and individual pouch of swabs. (The lot
number for A2 is embossed into the right side of the pouch. All other lot number's are embossed into the

Medtronic, Inc. (NYSE: MDT) today announced that it has identified an additional 396 monophasic
LIFEPAK® 500 automated external defibrillators (AEDs) to be included in a voluntary recall originally
announced on Feb. 25, 2005. The
affected AEDs may continue to display a "connect electrodes" message and may not analyze the patient’s
heart rhythm even when the electrodes are properly connected. Failure to analyze the patient’s heart
rhythm will inhibit defibrillation, if it is needed. Read

Pfizer Follows Up On February, 2005 Recall of
One Lot of Epilepsy Medicine - April 22, 2005 -
Pfizer Inc voluntarily recalled one lot (40,000 bottles) of 100 mg capsules of its epilepsy medication,
Neurontin, after a manufacturing mechanical failure resulted in some bottles containing empty or partially
filled capsules. Pfizer said only 100 mg strength capsules from lot #15224V --distributed in October and
November, 2004 -- are included in the recall. The production lot was distributed only in the United States.
No other Neurontin lots were affected. The company said it is possible that patients taking Neurontin to
control epilepsy could experience seizures from a missed dose of the product. Read

Purina Mills, LLC, is voluntarily recalling a specific lot of 50-pound bags of Antlermax Deer 20 deer feed.
The affected product was manufactured in Lubbock, Texas, February 15, 2005, and sold after that date. It
was shipped to seven feed and supply retail stores in west Texas and distributed in west Texas and eastern
New Mexico. The specific packages within the lot included in this limited recall have the lot number
5FEB15LUB1 printed on the sewing strip of each bag. Laboratory testing found a potentially harmful level
